About this role
The role is for a Navigator who will continue individualised case management and client navigation for people enrolled at community service centres, handling about 400 clients each. The position involves maintaining a reconciled centre client list, arranging follow-ups, supporting treatment continuity, and coordinating referrals to HIV, TB, maternal and child health and other services with the centre clinical team. It requires linking clients to approved services, using consent-based communication, and protecting client identity, with potential home visits only when appropriate and agreed. The job emphasises addressing barriers such as stigma and linking clients to psychosocial or safeguarding support, while tracking referrals and documenting outcomes without duplicating caseloads. Prior experience in case management, peer navigation or community service centres is needed, with a preference for those who have previously served as case managers at community service centres. Knowledge of local language and catchment area is desirable, and the role requires collaboration with facility and community teams and a commitment to confidential, quality health services.
